EIGHTEENTH ANNUAL HUMAN PERFORMANCE/ROOT CAUSE/TRENDING/SELF ASSESSMENT/OPERATING EXPERIENCE CONFERENCE

Mission: to provide a collaborative setting for participants to share concepts and best practices relating to continuous improvement initiatives – including human performance improvement, data analysis, root cause analysis, corrective action and other organizational effectiveness assessment tools and techniques.

The 2012 HPRCT Conference/Workshop Planning Team is searching for abstracts that provide first-hand accounts of work practices that have yielded positive results in Human Performance & Observation (HU), Root Cause (RC), Corrective Action, (CA) Trending (T), Operating Experience (OE), and Self Assessment (SA) to industry professionals. The intent is to provide information to the 18th HPRCT Conference in a format that allows other practitioners to implement the benchmarked good practices.

Instructions for submittal:

Please complete the following instructions by downloading the PDF form here and mailing it back to the addresses provided.

1.Choose a topic from one of the tracks above or propose a subject.
2.Decide on the format:
Manuscript/Presentation: An oral PowerPoint presentation (The presentation should be 30-45 minutes in length) covering the key points of the presentation that will be included in the conference proceedings.
Case Study: A presentation to the conference or breakout session of a completed or in-progress innovative program.
3.Prepare a 200-400 word summary describing the topic.
4.At the end of the summary, indicate the selected format: Manuscript/Presentation or Case Study.
5.Indicate area (Human Performance and Observations, Root Cause Analysis, Corrective Action, Trending, Self-Assessment, or Operating Experience).
6.Include your name, title, company, address, telephone, fax, and e-mail at the bottom of the page.
7.Send the summary in Microsoft Word or Adobe PDF format by December 16th, 2011 to:

Human Performance
  • Have you developed new tools or a new practice that has improved human performance?
  • Have you implemented new tools to improve the effectiveness of your organization?
  • Have you conducted recent research that you would like to shall?
  • Are there human performance issues or problems that you have recently resolved?
Root Cause
  • Are you including a Safety Culture assessment as part of your root cause/apparent cause investigations?
  • Have you experienced new learning from an event?
  • Have you improved the quality of corrective actions from events?
  • Have you developed effective ways of assessing the effectiveness of corrective actions?
Self-Assessment
  • Have you used self-assessments to make improvements in performance?
  • Have you developed improved methods for targeting areas in need of self-assessments?
Operating Experience
  • Have you used operating experience to prevent undesirable outcomes?
  • Have you developed improved methods for gathering and disseminating operating experience information?
Trending
  • Have you used trending data to identify and address a problem area?
  • Do you have effective Key Performance Indicators?
